Deposits and certificates of insurance
coi information for large orders
Please note that our default policy for new renters and orders of equipment with replacement values over $500 will require a Certificate of Insurance (COI) or a hold on a credit card as a deposit. If you have a COI, send it to rentals@eclipsefilmsupply.com prior to pickup. If booking through the website, it will automatically place a security deposit hold on your card. If the amount is too high and you can’t reserve the gear, please contact us and we can book on your behalf. If you have a COI and had a hold placed on your card, let us know and we can release the security deposit hold on your credit card after we receive the COI.
Our COI info: A Certificate of Insurance (COI) should be provided listing “Occulus Films, LLC”, with a business address of 7600 N 16th St, Suite E115, Phoenix, AZ 85020. We require listing as an “additional insured” for liability and a “loss payee” for equipment or property damage.
terms of service / rental contract
All renters are required to agree to our terms of service / contract for rental. If ordering online through our website, you will acknowledge and agree to this using a checkbox during your checkout process. If ordering by phone or email, we will send you a combined quote and contract which we require you sign and date, and send to our email rentals@eclipsefilmsupply.com prior to gear pickup.
how we handle deposits - small orders or no insurance
For orders with equipment replacement value under $3500, we will process a hold on your credit card which will be released upon return of all equipment in good condition. Any damage, theft, or loss will be assessed and an amount of the security deposit kept accordingly.
Orders valued over $3500 must generally provide rented equipment insurance coverage.
identification and credit cards
For new customers, the credit card provided must match your driver’s license. For returning renters we will accept company cards and generally provide more flexibility. All renters are required to provide valid photo identification. We make a copy or photo of each renter’s valid ID and credit card, front and back, for our records.
